Job Description :

Center for Family Development (CFD) seeks to find a full-time Case Manager. The Case Manager will work as a member of a multi-disciplinary team providing assessment, individual planning, street outreach, and intensive case management. The team will coordinate with community resources to provide access to address all needs, including substance use treatment, medication management, physical health treatment, mental health treatment, benefits and entitlements, appropriate and affordable housing, employment and educational opportunities, and supports for basic needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Provide outreach in the community to locate and engage patients in primary care.
2. Assess whether patients have ability to attend appointments or need home-based care.
3. Engage referred individuals and collect information regarding individuals’ history and current presentation through a combination of interviewing, records review, and consultation.
4. Work with community agencies and programs to broker services for referrals as identified on the patient’s plan.
5. Participate in staff meetings, huddles, and case conferences with team members to utilize interdisciplinary expertise and facilitate individual service plans.
6. Monitor and assure that the health and safety needs of individuals are met.
7. Maintain required records.
8. Work within scope of training and experience and seek consultation when appropriate.
9. Assist patients in improving health outcomes, including social determinants of health.
10. Drive personal vehicle to meet patients in the community and attend appointments.

SKILLS & ABILITIES:
1. Basic knowledge of Motivational Interviewing, Stages of Change, and Harm Reduction.
2. Demonstrated knowledge of community and social service agencies, including health services (substance use, mental health, and physical health), housing, employment, benefits, and other social services.
3. Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.
4. Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.
5. Ability to set boundaries, resolve interpersonal conflict and de-escalate issues.
6. Dependable, able to work under pressure; receptive to change, willingness to learn, cooperative approach to problem-solving.
7. Experience working with homeless individuals and providing street outreach preferred.

Eligibility as a Qualified Mental Health Associate (bachelor’s degree in a behavioral sciences/mental health field, or a combination of at least three year’s relevant work, education, training or experience) required.

Annual salary dependent on experience. Eligible for medical, dental, and vision benefits (eligible on 1st of the month after hire), paid time off (PTO) including sick time, and paid holidays.

Successful completion of background check required upon hire.

Pay: $43,000.00 – $50,000.00 per year
